Marquis MD provides concierge and membership-based medical and wellness services. Membership fees may include enhanced access, coordination, wellness, scheduling, administrative support, and other non-covered amenities. Membership fees do not replace insurance coverage and do not guarantee reimbursement by any insurer, Medicare, Medicaid, or other third-party payor.

Covered medical services may still be billed to insurance where permitted by applicable law and payer rules. Membership services may vary based on provider availability, medical appropriateness, patient eligibility, and applicable law.

Your Access to and Control Over Information

You may opt out of any future contacts from us at any time. You can do the following at any time by contacting us via the email address or phone number given on our website:

• See what data we have about you, if any.
• Change/correct any data we have about you.
• Have us delete any data we have about you.
