It also helps physicians evaluate existing heart conditions or investigate significant changes in cardiac findings. But here’s what matters for your health: a normal EKG result does not guarantee a healthy heart, and most healthy adults do not need one annually.

That distinction matters for adults who want to take a proactive approach to their health. Preventive medicine is not about performing every available test annually. It is about choosing the right evaluation at the right time based on symptoms, medical and family history, risk factors, previous findings, and the clinical judgment of a physician who understands the bigger picture.

What Is an EKG and How Does It Work?

An EKG is a quick, noninvasive test that records the electrical activity of the heart. Electrodes placed on the skin detect the electrical signals generated with each heartbeat. A computer translates those signals into wave patterns that allow a physician to evaluate the heart’s rate, rhythm, and electrical conduction.

You may see the test referred to as either an EKG or an ECG. Both abbreviations refer to an electrocardiogram.

Every heartbeat depends on a coordinated sequence of electrical activity. An electrical impulse initiates the heartbeat, travels through the heart, and causes its chambers to contract in an organized pattern. The heart muscle then electrically resets before the process begins again.

According to the American Heart Association, the test can provide information about how quickly the heart is beating, whether its rhythm is regular or irregular, and the timing of electrical impulses as they move through different parts of the heart.

The test only records electrical activity, making a standard resting EKG painless and noninvasive.

Why Would Your Doctor Order an EKG Test?

An EKG test can help a physician investigate symptoms, evaluate cardiovascular concerns, or monitor certain existing heart conditions.

A physician may consider an EKG when someone experiences:

  • Chest discomfort
  • Palpitations
  • A racing, pounding, or irregular heartbeat
  • Shortness of breath
  • Dizziness or lightheadedness
  • Fainting or near-fainting
  • Unexplained changes in exercise tolerance

These symptoms do not automatically mean that something is wrong with the heart; there may be cardiovascular and non-cardiovascular causes.

An EKG provides objective information that a physician can interpret alongside the member’s symptoms, physical examination, medical history, medications, and other findings.

The Mayo Clinic notes that an electrocardiogram can help diagnose irregular heart rhythms (arrhythmias) and may reveal patterns associated with a current or previous heart attack and other cardiac concerns.

New or severe chest pain, significant difficulty breathing, fainting, or symptoms that could indicate a medical emergency require immediate medical attention and a call to 911 rather than waiting for a routine EKG test or annual screening.

An EKG test may also be useful when a physician is monitoring an existing cardiovascular condition, investigating a change from a previous examination, or evaluating how certain medications affect the heart.

The value of the test therefore depends on context.

What Does an EKG Show?

An EKG can show the heart’s resting rate and rhythm and identify certain abnormalities in electrical conduction. It may reveal arrhythmias, conduction delays or blocks, and electrical patterns that suggest heart muscle injury or other cardiac changes. However, an EKG test does not provide a complete assessment of cardiovascular health on its own.

A physician examines multiple waves, intervals, and patterns on the tracing rather than simply classifying the result as “normal” or “abnormal.”

What an EKG Can Show

  • Resting heart rate and baseline rhythm
  • Certain irregular heart rhythms
  • Electrical conduction delays or blocks
  • Patterns that may suggest heart muscle injury or strain
  • Findings that may warrant further evaluation

What an EKG Cannot Guarantee

  • That a future cardiovascular event will not occur
  • That an intermittent arrhythmia will appear during the recording
  • A complete view of the heart’s anatomy
  • That the coronary arteries are free of disease
  • That a normal tracing means the heart is completely healthy

The distinction is important because cardiovascular care should not end with the question, “Was my EKG test normal?” A more useful question is what the result means in the context of your health, symptoms, previous findings, and overall cardiovascular risk.

What Can’t an EKG Tell You About Your Heart?

A standard resting EKG test provides a brief snapshot—about 10-30 seconds—of your heart’s electrical activity. This means it may miss intermittent or rare arrhythmias, cannot visualize the heart’s structure or coronary arteries, and cannot detect every form of heart disease.

For example, someone with occasional palpitations might have a completely normal EKG if their rhythm is regular during the recording. If an abnormal rhythm occurs only intermittently, a physician may consider longer-term rhythm monitoring or another evaluation depending on your symptoms and clinical circumstances.

Additionally, a normal EKG test does not rule out:

  • Coronary artery disease or plaque buildup
  • Heart valve problems or structural abnormalities
  • Heart muscle damage that hasn’t yet produced electrical changes

That’s why EKG results should always be interpreted alongside your symptoms, medical history, medications, and other findings—not viewed in isolation. One normal test result should never substitute for a thoughtful cardiovascular assessment.

What Happens During an EKG Test?

An EKG test takes about 5-10 minutes and requires you to lie still while small electrode pads attached to your chest, arms, and legs detect your heart’s electrical signals. The test is painless, involves no needles, and carries no risk.

During the test, you will lie still while the electrodes collect the information. The actual recording takes only a short time. Once complete, the electrodes are removed, and the resulting tracing can be reviewed by a physician.

What happens after the EKG depends on why it was performed. A reassuring result may require no additional cardiac testing, while a particular finding—or persistent symptoms despite a normal EKG—may prompt further evaluation.

How Often Should You Have an EKG?

There is no universal schedule for determining how often you should have an EKG. The appropriate timing depends on symptoms, medical history, cardiovascular risk, existing heart conditions, medications, previous findings, and your physician’s clinical judgment. For many healthy adults without symptoms, it’s not necessary to perform a routine annual EKG screening.

The U.S. Preventive Services Task Force recommends against using resting or exercise EKG to screen for cardiovascular disease risk in asymptomatic adults at low risk of cardiovascular events. For asymptomatic adults at intermediate or high risk, the Task Force states that current evidence is insufficient to determine the balance of benefits and harms.

These recommendations apply specifically to screening people without symptoms. They do not mean an EKG lacks value when a person develops symptoms, has an existing cardiovascular condition, has an abnormal examination finding, or presents another clinical reason for testing.

Should an EKG Be Part of Your Annual Physical?

Not necessarily. An annual EKG is not routinely recommended for every asymptomatic adult. Whether the test belongs in your annual evaluation should depend on your cardiovascular risk, symptoms, medical history, physical findings, previous test results, and other individual considerations.

It is easy to assume that adding another screening test makes an annual physical more comprehensive. More information can feel reassuring, particularly when cardiovascular health is a priority.

But preventive medicine works best when testing is connected to a meaningful clinical question. A thoughtful annual evaluation focuses not only on which tests can be performed, but on what has changed since the last visit.

Has your exercise tolerance changed? Are there new symptoms? Have your medications changed? Is there new information about your family history? Have blood pressure, laboratory findings, or other cardiovascular indicators shifted over time?

Those changes may be more clinically meaningful than simply repeating an EKG test because another year has passed.
